Business Associate Agreement
The account owner signs the BAA inside Survey Doctor. Your team cannot send assessments until it is signed. Read the agreement.
Role-based access and audit log
Roles control admin powers such as billing, exports, the team and the BAA. Every team member can see all patients and results at every location. Staff access to patient records and data exports, including PDF downloads, are written to an audit log.
Encryption
Records are encrypted at rest with AES-256 on managed PostgreSQL and in transit with TLS.

Do patients need an account?
No. Patients open the link, confirm their date of birth, and answer in the browser. Patients who want to see their results later can claim an account and consent to sharing.

Can a PHQ-2 or GAD-2 score trigger the full assessment?
No. Survey Doctor does not send a second assessment based on a score. Review the PHQ-2 or GAD-2 result, then send the full PHQ-9 or GAD-7 when you decide it is needed.
Does it connect to our EHR?
There is no direct EHR integration. Owners export records as CSV or JSON, so check what your EHR can import before you plan a transfer.
