About us

Survey Doctor is a mental health survey platform operated by TeleEMG, LLC. It is used by clinicians who want to measure how patients are responding to treatment, and by individuals who want to track their own mental health over time.

We publish validated assessments such as the PHQ-9, GAD-7, DASS-21 and PCL-5. Every questionnaire is transcribed from its official published source, item for item, and scored using the method the instrument's authors defined. We only publish instruments that are free to use, either in the public domain or available with attribution.

Practices send assessments by email, SMS or a QR code at the front desk. Results are scored on arrival and land in the patient's chart. When a healthcare organization uses Survey Doctor, we act as a HIPAA Business Associate and sign a Business Associate Agreement before any survey goes out. Patient data is held on managed PostgreSQL infrastructure with AES-256 encryption at rest and TLS in transit, and every access to it is written to an audit trail.

The software grew out of our own practice. We built it because tracking symptom scores on paper was slow, easy to lose, and impossible to chart over time. If you have a question about the platform or an instrument, get in touch.
