Delete your account

Last updated October 14, 2026

You can delete your personal Survey Doctor account yourself in Settings, in the iOS app or on survey.doctor. Survey Doctor is published by TeleEMG, LLC. If you cannot sign in, send a request through the contact form and we will confirm it with the email address on the account before deleting anything.

How to delete your account

Once you confirm, the account is closed right away and cannot be restored.

  1. Sign in, open your account, and choose Settings.
  2. Under Danger zone, choose Delete account.
  3. Type the email address on the account, enter your current password, and confirm.

If your account has no password, the dialog asks you to sign out and sign back in with the code we email you. Then delete the account the same way.

What is deleted and what remains

Deletion removes or de-identifies the data held only for your personal account: your profile, your own assessment responses and reminders, and your settings. Open checkout sessions are closed and an active subscription is cancelled. Limited audit, security, backup, billing, or legal records may remain for the periods described in the privacy policy.

Records that a clinic controls stay with the clinic. If a healthcare organization uses Survey Doctor for your care, deleting your personal account removes the link between your account and the clinic and ends your sharing consent. The clinic's medical records are not deleted; ask the clinic directly about those.

If you cannot sign in

Use the contact form, choose Technical support, and tell us the email address on the account and that you want it deleted. We reply to that address to confirm the request before deleting anything, so that nobody else can delete your account.
